NTSB formally determined that pilot fatigue and inadequate crew training were to blame,
but subsequent investigation revealed that there was a good chance the crash could have
been averted at the minimum safe altitude warning (MSAW) system been operating at the
time of the crash. Previous crashes also had been blamed on pilot error, and a separate
investigation by the FAA resulted in changes to the MSAW system nationwide.
